A classic, low-level AD&D module by Gary Gygax.
T1 Village of Hommlet by Gary Gygax
The sequel was delayed for many years, and finally released as supermodule
T1-4 Temple of Elemental Evil rather than
the originally-planned "T2". Even before the T2 decision, module codes
WG1 and WG2 were assigned to Village of Hommlet and
Temple of Elemental Evil respectively, but were dropped for unknown
reasons (thanks to Ian Malcomson for this info).
T1-4 contains an abbreviated (and slightly updated) version of
T1 Village of Hommlet.
Some contributors have reported 3-hole-punched versions of these modules.
TSR seems to have done this at random intervals with several different early
modules; the presence (or lack of) 3-hole punching is not, by itself, indicative
of a different printing.
